FINANCIAL TRACKING FORM FOR PAYMENTS

Dear Student Cooperative Degree Program Participant.

Here is a safe, old fashioned, way to make your degree program payments.

POSTAL MAIL PAYMENTS

Send a check or money order that is traceable

1. Fill out the information in the form below.

2. Write a check payable to the Institute of Global Education and postal mail it with an appropriately completed copy of the form.

Keep hard copies of payment and contracts. You may need these permanent records such as cancelled checks, study contracts or applications for graduation.

TO MAKE A PAYMENT VIA POSTAL MAIL:

Hard copy and submit two copies of the form below plus a self addressed stamped envelope for your receipt, with a payment made by a check or money order made out to Institute of Global Education.

To assure that your payment is properly acknowledged, this form should accompany payments of any kind.
